You can use this simple wording for a Saddhāporshi / Sādhāporasi fast with an allowance for medicine:
“Aaj meṅ Sādhāporasi kā upvās rakhtā / rakhtī hūṅ. Mātra āushadhi (medicine) ke liye jal lene ki anumati rahegi.”
If you want a more formal Jain-style vow, you can say:
“Main Sādhāporasi vrat svīkār kartā / kartī hūṅ. Is vrat mein bhojan ka tyāg rahega, aur keval āushadhi ke liye jal grahaṇ karne ki anumati rahegi.”
A very short version is:
“Sādhāporasi vrat with medicine-water only.”
